Visa & Immigration Medicals

We need this form before your visa medical appointment. The questions help us prepare the correct examination, paperwork, and any tests required for your destination country. Getting this right first time saves you a return visit - so please take a few minutes to fill it in as accurately as you can.

Requirements can vary by visa category even within the same country.
Medical certificates have validity periods (typically 3-6 months). We need to ensure yours will still be valid at submission.
If you have a letter or document from the consulate specifying what's needed, this helps us prepare everything in advance.
This includes diabetes, hypertension, heart disease, asthma, epilepsy, or any other ongoing condition.
TB screening is required for most visa medicals. A history of TB doesn't necessarily prevent visa approval.
A positive result does not necessarily prevent visa approval - the doctor can advise.
If a previous X-ray showed something abnormal, the doctor needs to know.
Some visa medicals require a mental health fitness declaration.
Some medicals require a declaration that the applicant is free from drug addiction.
If you have your vaccination record or red book, please bring it to the appointment.
BCG can affect TB skin test results. Look for a small round scar on your upper left arm.
Yellow fever vaccination is MANDATORY for entry to many African countries (Nigeria, Kenya, Ghana, Tanzania) and some Latin American countries. The ICVP (yellow card) is proof. We can provide the vaccination if needed.
Some countries still require proof of COVID-19 vaccination.
Legalisation = authentication by the UK FCDO and/or the relevant embassy. This is an additional step after the medical. We can advise on the process.
Some non-English-speaking countries require a certified translation. We can recommend translation services.
Visa medicals must be done face-to-face. You can have a call first to discuss requirements.
Family members can often be seen together.
