Ivermectin cream (Soolantra) is the recommended first-line topical treatment for the papules and pustules (spots and bumps) of rosacea. It has both anti-parasitic action (against Demodex mites, which are found in higher numbers in rosacea-affected skin) and anti-inflammatory properties. Applied once daily, it typically shows improvement within 4 weeks.
