What should I bring?

Upload a scanned medical report / certificate of good health with the online application at eservices.iss.gov.gy. A UK GP letter on headed paper confirming general fitness and absence of serious communicable disease, dated and signed with GMC number, should be fit for purpose.

Is there an official form?

There is no official form we can reliably link for Guyana. We draft the doctor's certificate to the format your Guyana embassy or High Commission in London, or your sponsor, specifies. If they give you a blank form, bring it and we complete it.
