What does the appointment involve?

A Namibia visa medical generally involves the following, based on Namibia's current requirements as we understand them today.

  • Examination: A medical report (form 3-1/0003) and a radiological report (form 3-1/0004), both required

  • Bloods: Any tests the forms ask for; additional charge, confirmed at consultation.

  • Chest X-ray: For TB screening. Referred to our imaging partner; additional charge.

  • Other tests: Radiological report (form 3-1/0004)

  • Vaccinations: Not part of the requirement; yellow fever only if arriving from a risk country. We do not provide yellow fever vaccination; please arrange it separately.

  • The report: Completed on the destination's own form, an embassy template, or a covering report we draft, with all signatures and stamps included.

  • Requirements and forms are your responsibility. Visa medical requirements change and embassies do not always announce it, so you must check the current requirements directly with the Namibia embassy or High Commission in London, obtain the correct forms, and satisfy yourself that you have exactly what your application needs. We will help wherever we can, but confirming what Namibia requires is yours to do.

What does Namibia ask for?

Namibia's Ministry of Home Affairs, Immigration, Safety and Security asks for two medical forms with Employment Permit, Permanent Residence Permit and Student's Permit applications. They are the medical report (form 3-1/0003) and the radiological report (form 3-1/0004), which covers the chest X-ray. Any document not in English must be translated by an official translator. A police clearance certificate is also needed, and this is separate from your medical. Requirements and forms can change, so please check the current requirements and form with the Namibian High Commission in London or your agent before you book.

What should I bring?

Obtain and complete Namibia's forms 3-1/0003 and 3-1/0004 (likely via the Namibian employer/sponsor or MHAISS); arrange a chest X-ray with an imaging partner. Confirm UK-completion acceptance with MHAISS or the High Commission London.

Is there an official form?

Namibia's Ministry of Home Affairs publishes the medical report (form 3-1/0003) and the radiological report (form 3-1/0004) on its e-services portal; please bring the current versions. We draft the doctor's certificate to the format your Namibia embassy or High Commission in London, or your sponsor, specifies. If they give you a blank form, bring it and we complete it.
