Pioglitazone is a thiazolidinedione (TZD) - the only class of diabetes medication that directly reduces insulin resistance, the underlying driver of type 2 diabetes. It’s fallen out of fashion somewhat due to side effects, but remains a useful option when other treatments aren’t suitable or effective, particularly in patients with non-alcoholic fatty liver disease.
